Long term condition monitoring- We offer appointments to monitor any long term conditions you may have. It is important to monitor these conditions, especially if you take medication where the dose needs to be controlled.You will be invited for a review if you have one or more of the following conditions :

• Asthma
• Chronic Obstructive Pulmonary Disease
• Diabetes
• Chronic Kidney Disease
• Heart Failure
• Coronary Heart Disease
• Atrial Fibrillation
• Hypertension

• Blood pressure monitoring (including 24 hour blood pressure monitoring)

• Simple wound dressings

• Phlebotomy (bloods) services including warfarin monitoring
